Health passport

Our health passport gives those with a disability or a health condition which impacts their job, the opportunity to talk through their individual needs and any flexibility needed with their manager. Should you move into another role the passport goes with you so there is no need for repeat conversations, unless your needs change.

How to access

Staff can access the passport as a part of the wellbeing at work toolkit on the Trust intranet. The link is only accessible to staff members logged into the Trust network.
